New Mexico Medicaid Plans
All Medicaid plans require a referral. Please have your provider fax it to us at (910)933-7931.
A note about Medicare (part B only)
Medicare has extensive restrictions about who can refer and for what.
- Medicare only covers nutrition (in a private practice setting) for diabetes or kidney disease.
- The referral must be from or cosigned by an MD or DO, per Medicare rules, for medical nutrition therapy. Please have them fax it to (910)933-7931.
- Dual Medicare/Medicaid Plan: These plans follow the Medicare guidelines.
Oregon Health Plan (OHP)
I am an enrolled OHP provider but not contracted with CCO networks.
- A referral from your medical provider for medical nutrition therapy (MNT), including diagnoses related to how your condition affects your nutrition (e.g., limited intake, GI symptoms, weight changes), is required.
- Please have your provider fax the above information to us at (910) 933-7931.
Self-pay
We offer discounted self-pay options for returning clients.
- Our standard rate is $210/hour for initial visits and $180/hour for follow-up visits.
- Recipe Analysis, Customized Meal Plans, Recipe Makeovers, Extended Appointments, In-Depth Diet Analysis $180/hour
- Discounted Package Rate: If you purchase 2 or more hours at once, we apply our package rate or 150/hour.
- “E-visits”: If your visits are scheduled more than 10 days apart, we bill for secure messaging to answer patient-initiated health-related questions. Not all messaging qualifies as an “e-visit.” You are welcome to ask us non-health-related questions anytime without being billed, or follow-up questions within 10 days of your visit.
- We also offer professional consultation and nutrition services on a fee-for-service basis, including menu development, staff training, and practical education designed to support accessible, low-burden nutrition care in disability and community-based settings.
- All fees will be disclosed and agreed upon in writing before commencing services. If I’m not contracted with your insurer, a superbill can be provided upon request for possible reimbursement. Payment is due at the time of service unless other arrangements are made in advance.
Referrals
Please have your provider make the referral to More Spoons LLC and fax the referral to (910)933-7931 with your most recent visit note, growth carts/vitals, recent labs, and ICD-10 diagnosis codes.
