Insurance and Fees

Insurance plans often cover nutrition consultations with a dietitian at no cost to you!

Coverage and network participation depend on the member’s plan and location.

Telehealth consultations available for clients located in New Mexico, California, Colorado, Oregon, Washington, New York, Texas, and Arizona.

New Mexico Medicaid Plans

New Mexico Medicaid Plans:

• NM Medicaid plans only cover nutrition for individuals under 21 and pregnant adults.
• Medicaid does not cover weight management nutrition.

• Medicaid plans require a referral. Please have your provider fax it to us at (575) 334-0201.

A note about Medicare (part B only)

We currently accept Medicare in New Mexico. However, Medicare has extensive restrictions about who can refer, for what, and where.

• Medicare only covers nutrition for diabetes or kidney disease.
• The referral must be from or cosigned by a MD or DO per Medicare rules for medical nutrition therapy.

Oregon Health Plan (OHP)

I am an enrolled OHP provider but not contracted with CCO networks. In some cases, services may be covered with prior authorization.

To request approval, you will usually need:

• A referral from your PCP or managing provider for medical nutrition therapy (MNT), including diagnoses and how your condition affects your nutrition (e.g., limited intake, GI symptoms, weight changes)
• Documentation of medical necessity, explaining why nutrition therapy is needed
• Out-of-network justification, describing why in-network care is not appropriate or available and the need for specialized support.

Self-pay

We offer discounted self-pay options for multiple visits.

• Our standard rate is $210/hour for initial visits and $180/hour for follow-up visits.
• Discounted Package Rate: If you purchase 2 or more hours at once, we apply our package rate, which averages 27% savings.
• Check-in Appointments, Recipe Analysis, Customized Meal Plans, Recipe Makeovers, Extended Appointments, In-Depth Diet Analysis $180/hour
• If your visits are set more than 10 days apart, we bill for secure messaging that answers patient-initiated health-related questions. Not all messaging qualifies as an “e-visit.” You are welcome to ask us non-health-related questions anytime without being billed, or follow-up questions within 10 days of your visit.
• All fees will be disclosed and agreed upon in writing before commencing services. If I’m not contracted with your insurer, a superbill can be provided upon request for possible reimbursement. Payment is due at the time of service unless other arrangements are made in advance.
• We also offer professional consultation and nutrition services on a fee-for-service basis, including menu development, staff training, and practical education designed to support accessible, low-burden nutrition care in disability and community-based settings.

Referrals

Please have your provider make the referral to More Spoons LLC and fax the referral to (910)933-7931 with your most recent visit note, growth carts/vitals, recent labs, and ICD-10 diagnosis codes.
